´óÏóÊÓƵestablished the Surveys on Patient Safety Culture^® (SOPS^®) Hospital Database as a central repository for survey data from hospitals that have administered the ´óÏóÊÓƵpatient safety culture survey instrument and choose to submit their survey data to the ´óÏóÊÓƵSOPS Hospital Database. The Database serves as an important resource for patient safety culture improvement. Participation in the SOPS Databases is free and open to users of the surveys to voluntarily submit their data to the appropriate database. In addition, organizations that administer any of the supplemental items with the companion SOPS survey can now also submit data to the appropriate database. Participating organizations receive a customized feedback report with their own results compared to the Database results. The SOPS Hospital Database contains data voluntarily submitted by participating hospitals and is not representative of all U.S. hospitals.

• Participating in the SOPS Hospital Database—The next data submission dates will be announced in 2025. Hospitals may submit data from the:
  • SOPS Hospital Survey, Version 2.0.
  • SOPS Health Information Technology Patient Safety Supplemental Items.
  • SOPS Workplace Safety Supplemental Items.
  • SOPS Value and Efficiency Supplemental Items for the Hospital Survey. 
• Using the Data for Research—Provides information and application instructions for researchers interested in obtaining de-identified and hospital-identifiable data files from the database to conduct research.

SOPS Hospital Survey Current Database Report Results

2024 Hospital 2.0 Survey Database Report (PDF, 3.7 MB); Executive Summary (PDF, 1.3 MB); Appendixes (PDF, 1 MB)—Presents data from 445 U.S. hospitals that administered the survey and submitted data. The report consists of a narrative description of the findings and two appendixes, presenting data by hospital characteristics and respondent characteristics for the database hospitals.

SOPS Hospital Supplemental Items Current Results

2024 Results for the ´óÏóÊÓƵSOPS Workplace Safety Supplemental Items for Hospitals (PDF, 1 MB); Executive Summary (PDF, 509 KB); Appendixes (PDF, 1 MB)—Results for the Hospital Workplace Safety Supplemental Items include data submitted to the 2024 Hospital Database for a total of 61,767 providers and staff in 94 hospitals.

Pilot Study Results from the 2017 ´óÏóÊÓƵSOPS Health IT Patient Safety Supplemental Items for Hospitals (PDF, 996 KB)—Presents data from 44 U.S. hospitals that that administered and submitted data for more than 9,000 providers and staff in 2017. The report consists of a narrative description of the findings and an appendix explaining calculations.

Results From the 2014 Pilot Study of the ´óÏóÊÓƵSOPS Value and Efficiency Supplemental Items for the Hospital Survey (PDF, 1 MB)—Presents data from 47 U.S. hospitals that administered and submitted data for more than 4,000 respondents in 2014.
